Position Summary
Under direct supervision, the My 2 Aunties (M2A) Home Visitation Program Case Manager works closely with Native American families who live within the defined service area who are identified as at risk and will provide in home-visiting services to prevent the disruption of the family and to help maintain the family by identifying the family’s needs, including substance use risks, prevention services, intervention during crisis, and follow-up. Maintain confidentiality of all privileged information.-
Associate’s Degree preferred and 6 Months of experience in a relevant family services field and/or equivalent combination of education and experience.

Accepts casework referral and case assignments from supervisor.

Receives case referrals to prevent family risks of maltreatment, substance use and family disruption.

Assists parent(s) in family life skills, parenting skills and related considerations.

Encourages parental coaching in early child development.

Advocates school attendance.

Performs home visits working directly with families.

Monitors and maintains case record files on client’s progress.

Reports suspected child abuse and neglect.

Provides reports and responds to supervision by the Program Coordinator.

Understands program policies and procedures of the program and pertaining professional standards.

Performs other duties as required.

Knowledge of Native American culture and values.

Knowledge of the general goals and purpose of M2A program

Knowledge of the general goals and purpose of the family home visitation programs, and the services to be provided.

Knowledge of policies and procedures governing family visitation and/or regulations pertaining to the program.

Knowledge and familiarity with community resources that provide ancillary services such as transportation, childcare, housing, training, and health services.

Knowledge of records management procedures.

Ability to communicate effectively in the English language both verbally and in writing using proper spelling, grammar, punctuation, and basic arithmetic.

Knowledge in budgeting, eligibility determination for services, and direct entry of data into a computer-based program.

Ability to interact and maintain good working relationships with individuals of varying social and cultural backgrounds, employees and officials.

Ability to gather data, compile information and prepare reports.

Ability to be organized and detail oriented while handling multiple tasks and meet deadlines in a fast-paced environment.

Skill in operating business computers and office machines, in a Windows environment.

Skill in analyzing problems, projecting consequences, identifying solutions, and implementing recommendations.

While performing the duties of this job, the employee regularly is required to sit and walk; use hands/arms/fingers to reach, lift, handle, or feel; and talk or hear. The employee must have skills of mobility or assistive mobility. The employee must occasionally lift and/or move up to 25 pounds. The employee occasionally is required to operate a motor vehicle and have the ability to travel to other sites and training locations with possible overnight stays. Work Environment:
