Provides patient care utilizing the nursing process.

Screens the patient by obtaining preliminary health history

Makes an assessment of the patient’s condition recognizing the range of normal and the manifestations of common abnormalities.

Determines the nature and urgency of the patient’s condition and makes appropriate referrals to health care staff.

Screens and assesses patients who are not physically present, but call in by phone.

Determines the need to be seen and the urgency of the condition, arranges for necessary tests and procedures.

Develops a care plan which includes a nursing diagnosis and an assessment of learning and counseling needs.

Provides direct nursing care to patients, including measures designed to prevent complications and minimize disabilities.

Assists physician/nurse practitioner with diagnostic, therapeutic, and surgical procedures.

With appropriate order or protocol, administers oral, rectal, ophthalmic, otic and parenteral medications and immunizations.

Ability to perform venipuncture for introduction of intravenous (IV) therapy or therapeutic phlebotomy, maintain an IV line, administer IV medications and fluids, perform PICC, port or central line care, lab draw and infusions.

Operates a variety of medical equipment.

Provides appropriate wound care with order from physician/nurse practitioner (unless Certified Wound Care Nurse, then may initiate wound care).

Monitors stable phases of chronic illnesses, minor health problems, and emergency conditions in the absence of a physician/nurse practitioner.

Makes an assessment, orders tests per established protocols, interprets results, determines nursing care plan, and makes appropriate referrals.

Documents in the patient's health record observations, assessment, nursing interventions, therapeutic measures administered, and referrals for follow-up care.

Evaluates results of patient care provided; makes changes in procedures and approaches to care as necessary for quality assurance.

Ensures continuity of care by relevant health counseling, referrals for follow-up care, arrangements for appointments, and collaboration with other health care professionals.

Acts as a patient advocate regarding both preventive and therapeutic medical aspects in dealings with various health care providers, including clinic personnel and non-clinical personnel.

Takes into account medical implications as well as relationship between clinic or agency and community.

Makes home visits for the purposes of nursing, laboratory testing, medical follow-up, and health maintenance.

Assumes urgent public health or case management nursing responsibilities in the absence of a public health/ case management nurse.

Provides laboratory services.

Obtains laboratory specimens, performs laboratory test, prepares specimens for transport to reference laboratory.

Performs quality control procedures.

Maintains logs and records. Files and maintains reports.

Participates in determination, updating and development of policies, procedures, and resources required for consistent, comprehensive, and efficient health services.

Identifies areas of inter/intra-departmental conflict or breakdown in delivery of care and initiates discussion, planning, implementing and/or coordination of resolution and necessary changes.

Serves as focal point for intra-office coordination in order to ensure the smooth operation of the health care clinic.

Provides technical supervision and training for nursing and laboratory staff. Provides guidance, direction, and training to ancillary personnel.

Acquires, maintains and audits supplies and equipment for the clinic; prepares for specialty clinics by reviewing appointment lists, seeing that the work area is set up to accommodate the particular activity, etc., prepares and submits reports and records.

Participates in quality assurance audits and surveys.

Participates in development and revision of quality assurance standards; implements standards.

Collaborates with other health care professionals to develop standard clinic procedures.

Directs and implements nursing portion of emergency and safety planning.

Makes decisions on nursing and laboratory problems not covered by precedents, protocols, or established policies.

Assists in development of goals and objectives; collaborates with Director of Nursing/ DNP to set priorities for nursing staff.

Maintains personal nursing licensure, ACLS and CPR certification.

Assists Wellness/Public Health Nurse with employee health program with respect to immunizations, communicable diseases, and potential work-related disabilities.

Participates in general clinic planning, provides major input for nursing department.

Provides assistance to other departments as needed.

Provides teaching and counseling services to patients, families, and the community as a whole.

Fosters patient motivation and compliance in adhering to required routines by counseling and assisting patients to understand and accept implications of care, diagnosis, and treatment.

Performs or assists in developing and implementing programs to educate the community.

Orients new clinic staff.

Identifies the need for coordinates and conducts in-service training. Maintains professional knowledge and skills by attending appropriate in-service and educational programs. Keeps professional licenses current.

Uses background of education and experience, physicians orders, protocols, policy and procedure manuals, and professional standards, etc., which are not always applicable to every situation.

Consults with other health care providers on difficult or unusual problems.
